Jacquita earned her Doctorate of Nursing Practice at University of South Alabama. She focuses on individualized and holistic care. Her goal is to meet the patient where they are at to help meet their health care goals.

At Fountain Valley Health Care, we treat even the smallest members of the family! From infancy to elderly, we're here for examination, diagnosis, treatment and ongoing wellness throughout the lifespan. 

General Health

Not feeling well or just need an annual exam? We do it all! We also perform sports physicals and referrals for injuries. Whether you're sick or only need some routine maintenance, we're here for you!

Mental Health

Having issues with depression, anxiety, bipolar, schizophrenia? We are able to help with mental health as well as physical health needs. We are experienced with injectable medications and local resources for counseling. We have two Pyschiatric-Mental Health Nurse Practitioners on staff. 

Addiction Treatment

Struggling with substance use? We can help with Medication Assisted Treatment (MAT) for addiction. We offer Vivitrol, Suboxone, and Sublocade as well as detox medications. Come start a new chapter in your life in a safe, judgement free clinic. 

Telehealth Services

Sometimes there are obstacles to coming into a medical office, but help is still available. Our doctors can meet you over a HIPAA compliant connection. Same day appointments may be available!
